Quickstart Overview

Tirzepatide is a 39-amino-acid dual GIP and GLP-1 receptor agonist built on the GIP backbone with a C-20 fatty diacid moiety. It is the first 'twincretin' to reach clinical use and produced the largest weight reductions of any approved peptide in the SURMOUNT programme.

Units assume a U-100 insulin syringe (100 units = 1 mL). Higher doses mean larger volumes — consider 10 mg/mL (1.0 mL water per 10 mg) for doses ≥ 7.5 mg.

Frequency: Once weekly

Reconstitution Steps

  1. 1Bring vial to room temperature; swab stoppers.
  2. 2Add bacteriostatic water slowly down the glass wall.
  3. 3Swirl gently until clear.
  4. 4Label and refrigerate.

Important Notes

  • GI adverse events are comparable to or slightly lower than semaglutide at equivalent efficacy, attributed to GIP's anti-emetic signalling.
  • Same rodent thyroid C-cell boxed warning class as other incretins.
  • Greater reductions in triglycerides and liver fat than GLP-1 mono-agonists.
  • Lean-mass preservation remains a research focus; ~25% of weight lost is lean tissue in body-composition substudies.

How This Works

Tirzepatide binds the GIP receptor with affinity equal to native GIP and the GLP-1 receptor with roughly 5-fold lower affinity than native GLP-1, with biased signalling toward cAMP over β-arrestin recruitment. GLP-1 agonism drives glucose-dependent insulin release, glucagon suppression, delayed gastric emptying and central satiety. GIP agonism adds adipose-tissue insulin sensitisation, improved lipid buffering, and central effects that appear to blunt nausea. The combination produced 20.9% mean weight loss at 15 mg in SURMOUNT-1 and HbA1c reductions up to 2.4% in SURPASS.

Potential Benefits & Side Effects

Reported in research
  • ≈ 21% mean body-weight loss at 15 mg over 72 weeks (SURMOUNT-1)
  • HbA1c reduction of 1.9–2.4% (SURPASS programme)
  • Superior weight loss vs semaglutide in head-to-head SURMOUNT-5
  • Marked reductions in liver fat, triglycerides and visceral adiposity
  • Improvement in obstructive sleep apnoea (SURMOUNT-OSA)
  • Heart-failure outcome benefits (SUMMIT)
Possible considerations
  • Nausea (31%), diarrhoea (23%), vomiting (12%), constipation (17%) at 15 mg
  • Decreased appetite, dyspepsia, abdominal pain
  • Injection-site reactions, fatigue, hair loss
  • Gallbladder events; rare pancreatitis
  • Hypoglycaemia with concurrent insulin/sulfonylurea

Lifestyle Factors

  • Protein ≥ 1.6 g/kg and progressive resistance training to limit lean-mass loss
  • Smaller meals; stop eating at first satiety signal
  • Hydration and fibre intake to offset constipation
  • Monitor for rapid-weight-loss effects: gallstones, hair shedding, fatigue

Injection Technique

  1. 1Subcutaneous abdomen, thigh or upper arm; rotate weekly.
  2. 2Volumes above 0.5 mL benefit from slow 10-second injection.
  3. 390° insertion into pinched fold; hold 5–10 s after plunger depression.
